Creaking Handlebars/Stem

On my ride today I noticed that whenever i stand up on my pedals there is a creaking sound coming from my handlebar/stem area because all my weight is applied up there. What is a possible fix? I have tried tightening the stem bolt, but it didn't fix it. I really love the concept of having all of my front teeth intact and I am scared that this sound will lead to their demise. I am riding with Soma El Toro steel pursuit bars and a quill stem.

I know I have been looking

I know I have been looking stuff up for an hour and a half and no ones advice seems solid. I just didn't know if anyone here had personal advice. Also, I have a threaded headset and the top keeps loosening as I ride, what is a fic for that?

my bet is that it's your

my bet is that it's your headset. I've certainly heard creaking headsets before. that fact that your headset keeps loosening up adds supporting evidence. all the threaded headsets I've seen have a locknut, and thus should not loosen on their own. crank that top race down as far as it will go without the bearings binding, and then, holding that in place with an appropriate wrench, crank the locknut down onto it.
